Background
Alcohol use disorder (AUD) and schizophrenia (SCZ) frequently co-occur, and large-scale genome-wide association studies (GWAS) have identified significant genetic correlations between these disorders.
MethodsWe used the largest published GWAS for AUD (total cases = 77 822) and SCZ (total cases = 46 827) to identify genetic variants that influence both disorders (with either the same or opposite direction of effect) and those that are disorder specific.
ResultsWe identified 55 independent genome-wide significant single nucleotide polymorphisms with the same direction of effect on AUD and SCZ, 8 with robust effects in opposite directions, and 98 with disorder-specific effects. We also found evidence for 12 genes whose pleiotropic associations with AUD and SCZ are consistent with mediation via gene expression in the prefrontal cortex. The genetic covariance between AUD and SCZ was concentrated in genomic regions functional in brain tissues (p = 0.001).
ConclusionsOur findings provide further evidence that SCZ shares meaningful genetic overlap with AUD.
Randomized Controlled Trial of Point-of-Care Ultrasound Education for the Recognition of Tension Pneumothorax by Paramedics in Prehospital Simulation

Objective:
The primary goal of this study was to determine if ultrasound (US) use after brief point-of-care ultrasound (POCUS) training on cardiac and lung exams would result in more paramedics correctly identifying a tension pneumothorax (TPTX) during a simulation scenario.
Methods:A randomized controlled, simulation-based trial of POCUS lung exam education investigating the ability of paramedics to correctly diagnose TPTX was performed. The US intervention group received a 30-minute cardiac and lung POCUS lecture followed by hands-on US training. The control group did not receive any POCUS training. Both groups participated in two scenarios: right unilateral TPTX and undifferentiated shock (no TPTX). In both scenarios, the patient continued to be hypoxemic after verified intubation with pulse oximetry of 86%-88% and hypotensive with a blood pressure of 70/50. Sirens were played at 65 decibels to mimic prehospital transport conditions. A simulation educator stated aloud the time diagnoses were made and procedures performed, which were recorded by the study investigator. Paramedics completed a pre-survey and post-survey.
Results:Thirty paramedics were randomized to the control group; 30 paramedics were randomized to the US intervention group. Most paramedics had not received prior US training, had not previously performed a POCUS exam, and were uncomfortable with POCUS. Point-of-care US use was significantly higher in the US intervention group for both simulation cases (P <.001). A higher percentage of paramedics in the US intervention group arrived at the correct diagnosis (77%) for the TPTX case as compared to the control group (57%), although this difference was not significantly different (P = 0.1). There was no difference in the correct diagnosis between the control and US intervention groups for the undifferentiated shock case. On the post-survey, more paramedics in the US intervention group were comfortable with POCUS for evaluation of the lung and comfortable decompressing TPTX using POCUS (P <.001). Paramedics reported POCUS was within their scope of practice.
Conclusions:Despite being novice POCUS users, the paramedics were more likely to correctly diagnose TPTX during simulation after a brief POCUS educational intervention. However, this difference was not statistically significant. Paramedics were comfortable using POCUS and felt its use improved their TPTX diagnostic skills.

Three-dimensional (3-D) geometry can be described in many ways, with both a varying syntax and a varying semantics. As a result, several very diverse schemas and file formats can be deployed to describe geometry, depending on the application domain in question. In a multidisciplinary domain such as the domain of architecture, engineering, and construction, this range of specialized schemas makes file format conversions inevitable. The approach adopted by current conversion tools, however, often results in a loss of information, most often due to a “mistranslation” between different syntaxes and/or semantics, leading to errors and limitations in the design conception stage and to inefficiency due to the required remodeling efforts. An approach based on semantic web technology may reduce the loss of information significantly, leading to an improved processing of 3-D information and hence to an improved design practice in the architecture, engineering, and construction domain. This paper documents our investigation of the nature of this 3-D information conversion problem and how it may be encompassed using semantic web technology. In an exploratory double test case, we show how the specific deployment of semantic rule languages and an appropriate inference engine are to be adopted to improve this 3-D information exchange. It shows how semantic web technology allows the coexistence of diverse descriptions of the same 3-D information, interlinked through explicit conversion rules. Although only a simple example is used to document the process, and a more in-depth investigation is needed, the initial results indicate the suggested approach to be a useful alternative approach to obtain an improved 3-D information exchange.

Objective
To specify the principles, definition and dimensions of the new nutrition science.
PurposeTo identify nutrition, with its application in food and nutrition policy, as a science with great width and breadth of vision and scope, in order that it can fully contribute to the preservation, maintenance, development and sustenance of life on Earth.
MethodA brief overview shows that current conventional nutrition is defined as a biological science, although its governing and guiding principles are implicit only, and no generally agreed definition is evident. Following are agreements on the principles, definition and dimensions of the new nutrition science, made by the authors as participants at a workshop on this theme held on 5–8 April 2005 at the Schloss Rauischholzhausen, Justus-Liebig University, Giessen, Germany.
ResultNutrition science as here specified will retain its current [classical] identity as a biological science, within a broader and integrated conceptual framework, and will also be confirmed as a social and environmental science. As such it will be concerned with personal and population health, and with planetary health – the welfare and future of the whole physical and living world of which humans are a part.

Summary
Scope of the problem
Patients manifesting abnormal behavior are common in emergency departments (EDs). They represent one of the most challenging classes of patients the emergency physician must treat. The causes of abnormal behavior are exceedingly diverse and require physicians to maintain a high level of vigilance to determine whether an underlying medical disorder exists. In 1998, it was estimated that nearly 4% of the approximately 100.4 million ED visits in the US were for behavioral problems. Many of these patients present “for medical clearance” prior to an intended psychiatric hospitalization. It is important that these patients be treated with the same sensitivity as every patient in the ED. “Medical clearance” should include a comprehensive medical evaluation to identify any potential underlying medical problem that may be responsible for the changes in behavior.
Pathophysiology
The physiology of behavior represents a complex interplay of human physiology and the environment in which it exists. Historically, changes in behavior have been classified as being of functional (psychiatric) or organic (medical) etiology. These classifications are dated, as neuropathophysiologic mechanisms of psychiatric disease have advanced over the past decades. Examples include aberrations in neurotransmitter transduction in depression (serotonin), schizophrenia (dopamine) and Alzheimer's disease (acetylcholine). Pharmacologic therapy directed at modulation of these neurotransmitters has greatly advanced the treatment and prognosis of patients suffering with these illnesses.
History
Prior to obtaining the history, the safety of the patient and staff should be ensured.
Microscopic Characterization of Polycrystalline APCVD CdTe Thin Film PV Devices

Atmospheric pressure chemical vapor deposition (APCVD) is being studied as an alternative for large-area manufacturing of CdTe thin films. High efficiency research cells have been constructed, but the fundamental materials properties and limitations have not been fully explored. APCVD material is examined with several techniques and compared with close-space sublimation (CSS). Transmission and scanning electron microscopy studies show a similar morphology to CSS CdTe. However high resolution TEM scans show the formation of a disordered layer between the CdTe and CdS, and the removal of defects within some grain structures upon annealing. Cathodoluminescence shows electronic defect states localized to grain boundaries. A large concentration of trap states was also observed with deep-level transient spectroscopy that may correspond to hole traps found in lower amounts in other materials. The presence of traps was also indicated in impedance spectroscopy measurements. The latter studies indicate a high grain boundary resistance contributes to transport.
